 EDUCATION and/or EXPERIENCE

Masters in Social Work or Counseling/Rehabilitationtwo years experience working with children or adolescents.  Eligible for state licensing.

ESSENTIAL DUTIES AND RESPONSIBILITIES

 Provide weekly individual counseling and document same.

 Provide weekly group counseling sessions and document same.

 Treatment Plan review of residents (approximately two) at each weekly Team meeting.

 Provide crisis intervention as needed.

 Develop treatment plans and report monthly on progress to community funding sources, guardians, and other involved parties (P.O.s, G.A.L.s, etc).

 Serve as clinical liaison to referral sources and parents regarding resident progress and needs; to include attendance of FAPT meetings as necessary.

 Take part in weekly multi-disciplinary treatment team meetings.

 Develop and implement plans for individual resident discharges.

 Coordinate closely with other staff to ensure the appropriateness of treatment modalities.

 Exercise sound judgment in proposing changes to resident treatment plans.          

 Maintain high quality documentation of all case work, completed in a timely manner and consistent with outlined program policies and licensure requirements.

 Maintain record of all services provided.

 Lead/participate in staff training and/or seminar presentations as requested.

 Provide supervision for post-graduate intern or LPC, LCSW, candidate as needed.

 DOCUMENTATION REQUIREMENTS & SCHEDULE

Initial Service Plan-                                                                         -w/in 24 hours of admission

Behavioral Support Plan-                                                                     -w/in 10 days of admission

Community Integration and Service Plan--                             -w/in 30 days of admission

Community Integration and Service Plan Update-             -every 30 days

Discharge Report-                                                                            -due 30 days after discharge

 

 SUPERVISORY RESPONSIBILITIES

The position may supervise an intern when assigned.
